Welcome to the office of Jones Eye Care and Surgery, Inc.

Thank you for choosing Jones Eye Care and Surgery with your vision care! Our health team and staff are dedicated to providing you and your family with the best possible medical care.

Full optical services are available at both locations.

This packet is your guide to our practice and policies.

Appointments

The office is open from:
8:00am – 5:00pm (Monday & Wednesday),
7:30am – 4:30pm (Tuesday & Thursday),
8:00am – 3:00pm (Friday).

Patients are seen by appointment only. Please understand that medical emergencies do occur and in these circumstances we ask for your consideration. If you cannot keep an appointment, we ask that at least 24 hour notice be given to the office. This makes it possible to schedule an appointment time for another patient.

Registration

Please bring the following with you to your appointment:

  1. Completed New Patient Forms
  2. Current Insurance Cards and Picture ID
  3. List of all current medications and pharmacy information
  4. Referral Forms (if required by your insurance)
  5. Your current glasses and any written prescriptions you may have
  6. Your contact lenses, case and any written prescriptions you may have
  7. Co-Pay, if applicable
Your eyes may be dilated during the exam, please remember that it usually takes several hours to wear off and may make driving difficult. If you do not feel comfortable and safe driving after dilation, we recommend arranging for someone to bring you home after your appointment.
